What changes the plan is the tissue underneath. Fat (pseudogynecomastia) often responds to liposuction alone. Firm gland (true gynecomastia) usually needs excision, and loose skin may need removal. A consultation with Dr. Mughni at EKAGRA Health, Dhanmondi, identifies which one you have.

Reviewed by Dr. Chowdhury Rashedul Mughni, BMDC reg. Treatment decisions depend on individual factors that can only be evaluated during a consultation with Dr. Mughni.

Male breast surgery is an operation that reduces an enlarged male chest, usually caused by gynecomastia, a benign enlargement of glandular breast tissue. Men across Dhaka and Bangladesh search for “man boob surgery” or “male breast reduction” and wonder whether these differ. This guide, reviewed by Dr. Chowdhury Rashedul Mughni, a plastic surgeon in Dhanmondi, explains the overlap and how he decides which approach fits.

Is male breast surgery the same as gynecomastia surgery?

Yes, in almost every case. Male breast surgery, male breast reduction and gynecomastia surgery all describe one operation that removes excess chest fat, gland or skin. The exception is surgery for male breast cancer, such as mastectomy, which is a separate oncological operation not covered here.

The American Society of Plastic Surgeons uses the terms interchangeably. Strictly, “gynecomastia” names glandular growth. A chest enlarged mostly by fat is pseudogynecomastia, and a mix of the two is common.

Nor is male breast surgery a smaller female breast reduction. A female reduction reshapes a breast that is meant to stay; a male operation aims for a flat chest contour. Both sit within breast surgery services at drcrmughni.com, but are planned differently.

At a glance: gland, fat, or both?

The tissue type, not the operation’s name, decides the plan.

Feature

True gynecomastia

Pseudogynecomastia

Mixed

What the tissue is

Glandular breast tissue

Fat, no gland growth

Gland and fat together

How it feels

Firm or rubbery disc under the nipple

Soft and evenly spread

Firm centre, soft surround

Effect of weight loss

Does not remove gland

Often reduces it

Reduces the fat part only

Usual surgical approach

Gland excision with liposuction (liposuction alone if mild and soft)

Liposuction alone

Liposuction plus gland excision

If skin is loose (all types)

Skin excision in marked cases

Skin excision in marked cases

Skin excision in marked cases

Scar

Small curved scar at the lower areola edge

Small puncture marks

Both

Durability

Recurrence uncommon once gland is removed and the cause treated

Fat can return with weight gain

Fat part can return with weight gain

When is liposuction alone enough?

Liposuction alone suits a chest that is mostly fat, with no excess skin and no firm disc behind the nipple.

Small cannula punctures keep scarring minimal. Published guidance also recommends liposuction alone for mild, soft glandular enlargement without excess skin (Simon grade I or IIa). For an overweight patient, reaching a stable weight first is sensible, because fat can return if weight rises later.

When does the gland need to be removed?

Gland excision is needed when a firm, rubbery disc sits behind the nipple, because published guidance removes firm, fibrous tissue directly rather than relying on suction.

The gland is usually removed through a small half-moon incision at the lower edge of the areola, where the scar blends with the darker skin. Gynecomastia present for more than a year tends to become fibrous, which makes it less likely to settle by itself.

In marked cases with sagging skin (Simon grade III and some grade IIb), some skin is removed as well, and the scars are longer. The grading system for gynecomastia explains these stages.

How Dr. Mughni decides which is right for a given patient

The decision starts with an examination, not a label: feeling the chest shows fat, gland or both, and any skin excess.

The consultation questions are practical. Is there a firm disc under the nipple? Is the skin tight or loose? Has weight changed? Could medicines, supplements or anabolic steroids be driving it? The answers decide whether liposuction, excision or skin removal is the honest recommendation.

Many men live with an enlarged chest for years before asking for advice, avoiding swimming or fitted shirts. Dr. Mughni describes how he opens those conversations:

“I make them realize that it’s totally normal and it’s treatable. In adults, a medicine or a condition such as liver, kidney or thyroid disease can cause gynecomastia, and treating the cause may reduce it. The common causes of male breast enlargement are covered separately.

A hard, fixed or growing lump, or one with nipple discharge, retraction or skin change, is not a cosmetic question. It needs medical evaluation, with imaging where indicated, before reduction surgery is discussed.

What are the trade-offs of each approach?

Every option trades scar against completeness. Liposuction leaves the smallest scars but can leave gland behind; excision removes gland but adds a scar.

Published surgical series report hematoma (a collection of blood) as the most frequent early complication. Other recognised risks include seroma (a fluid collection), contour irregularity, over- or under-removal, nipple numbness and visible scars. One UK single-surgeon series of 53 breasts reported complications in 22.6% of breasts, most of them minor.

ASPS guidance describes desk work within a few days after limited procedures and two to three weeks after extensive ones. A small Bangladeshi series reported about five days back to office work. A compression vest is worn day and night for about six weeks, and the chest feels swollen and tight beneath it before settling over months. The week-by-week recovery timeline covers the details.

Frequently asked questions

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Is male breast surgery the same as gynecomastia surgery?

Usually, yes. Male breast surgery, male breast reduction and gynecomastia surgery describe the same operation to flatten an enlarged chest. The plan differs by tissue: liposuction for fat, excision for firm gland, skin removal for marked sagging. What an examination finds matters more than the name.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Can exercise or diet get rid of gynecomastia?

Diet and exercise can reduce chest fat, so pseudogynecomastia often improves with weight loss. True gynecomastia is glandular tissue, and neither weight loss nor chest exercise removes it. Exercise builds the muscle underneath but leaves the gland in place. If a firm disc remains after weight loss, it is likely gland.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Does gynecomastia go away on its own?

Often, in teenagers. Pubertal gynecomastia settles in most boys within one to three years, so surgery is usually delayed at least 12 months. In adults it may improve once a causative medicine or condition is treated. Gland present for years tends to become fibrous and stay.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Is a lump under the nipple in a man cancer?

Most lumps under the male nipple are gynecomastia, which is benign. A lump that is hard, fixed or growing, or one that comes with nipple discharge, nipple retraction, skin changes or swollen armpit glands, needs prompt medical evaluation and possibly imaging. That assessment comes before any cosmetic surgery is considered.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Can gynecomastia come back after surgery?

Recurrence is uncommon when the gland is removed and the cause is dealt with. Remaining gland can grow again with anabolic steroid use or certain medicines, and fat can return with significant weight gain. Keeping a stable weight and avoiding known triggers protects the result over the long term.

</details>

The bottom line

Male breast surgery is one operation with different routes, chosen by what the tissue actually is. The answer comes from an examination, not a search term. The aim is to restore a chest contour a man is comfortable with. For treatment options, recovery and cost, the main gynecomastia treatment guide covers each stage. If you would like a personal assessment, a consultation with Dr. Mughni at EKAGRA Health is the place to discuss whether surgery is right for you.

This article is covered by our medical disclaimer.

References

1.
