A general breast surgeon or surgical oncologist removes a lump or the whole breast to treat disease, while a plastic surgeon works on the form of the breast. That work is either cosmetic, chosen to change appearance, or reconstructive, done to restore form or relieve physical symptoms.

Which breast surgery fits which concern?

The right procedure depends on the main concern: too little volume, too much weight, sagging, sagging with volume loss or breast loss after cancer.

Your main concern

Procedure usually discussed

What it changes

What it does not change

Breasts feel small or have lost volume

Breast augmentation

Adds volume and upper fullness

Does not lift a breast that has dropped significantly

Heavy breasts causing neck, back or shoulder pain

Breast reduction

Removes weight, lifts and reshapes

Leaves longer scars than augmentation

Breasts have dropped after pregnancy, breastfeeding or weight change

Breast lift (mastopexy)

Raises the nipple and reshapes tissue

Does not add volume on its own

Breasts have dropped and lost volume

Lift with implants

Lifts and adds volume

Carries the risks of both procedures

Breast removed or changed by cancer treatment

Breast reconstruction

Rebuilds breast shape

Does not replace cancer follow-up

Breast augmentation

Breast augmentation uses implants to increase breast size and improve upper-pole fullness. The detail on implants, results and preparation is on the breast augmentation guide, and the differences between silicone, saline, round and teardrop shapes are set out in a comparison of breast implant types.

Breast reduction

Breast reduction removes breast tissue, fat and skin to make heavy breasts smaller, lighter and higher. Many patients come with neck and shoulder pain, bra-strap grooves or skin irritation under the breast. A large systematic review of 95 studies (Gland Surgery, 2019) found improvement in physical symptoms, quality of life and self-esteem after reduction. For these women, the goal is comfort and function as much as appearance. For candidacy, technique and recovery, see relief from heavy breasts through reduction surgery.

Breast lift (mastopexy)

A breast lift raises the nipple and reshapes breast tissue that has descended. Surgeons grade this descent (ptosis) by where the nipple sits relative to the fold under the breast. Mild cases may need only an incision around the areola; more advanced cases need a vertical or anchor-shaped incision. See reshaping the breast after sagging or pregnancy.

Lift with implants

When the breast has both dropped and lost volume, a lift alone may leave the upper breast flat, and an implant alone may sit under a sagging breast. Combining both in one plan addresses volume and position together. In more severe cases, or when the nipple’s blood supply is at risk, the procedures may be done in two stages for safety.

Breast reconstruction after mastectomy

Breast reconstruction rebuilds the breast after part or all of it has been removed for cancer. It can use implants, the patient’s own tissue (a flap) or both. Breast cancer is one of the most common cancers in Bangladesh, and reconstruction is worth raising with the cancer team before or after mastectomy. For many women it restores shape, confidence and hope after treatment. A 2024 Cochrane review found that tissue flaps may give higher satisfaction, while implants may carry more long-term complications. The evidence is low certainty, and no single method suits every woman. Options are explained on reconstruction options after mastectomy in Bangladesh.

Can men have breast surgery?

Men with enlarged breast tissue usually have gynecomastia, a benign enlargement of gland tissue, fat or both. It is treated with liposuction, gland removal or both. The page on how male breast surgery differs from gynecomastia treatment explains the overlap, and the full gynecomastia surgery guide covers grades, recovery and results.

Who is a candidate for breast surgery?

A good candidate is healthy, a non-smoker or willing to stop, at a stable weight and realistic about the result and the scar.

The points Dr. Mughni looks at include:

• General health. Diabetes, high blood pressure and heart or lung conditions should be controlled before surgery.

• Smoking. Smoking reduces blood supply to healing skin. Active smokers are asked to stop before a lift or reduction.

• Stable weight. Large weight change after surgery changes breast shape. Planned weight loss is best completed first.

• Pregnancy plans. Many women choose to complete their family before a lift or reduction.

• Breast health. Any lump, nipple discharge or skin change is investigated first. Age-appropriate breast imaging may be requested before surgery.

• Expectations. Surgery improves shape and proportion. It does not produce a perfect or identical pair of breasts, and every operation leaves a scar.

How is breast surgery performed?

Breast operations are performed under anaesthesia in an operating theatre, through incisions placed where scars are least visible.

• Augmentation. The implant is placed through an incision in the fold under the breast, around the areola or in the armpit. It sits either under the breast gland or partly under the chest muscle.

• Reduction. Excess tissue and skin are removed. In most techniques the nipple stays attached to a living strip of tissue (a pedicle) that keeps its blood supply.

• Lift. Skin is removed and the breast tissue is reshaped and raised. The incision pattern depends on how far the breast has dropped.

• Reconstruction. Implants, a tissue flap or both are used, often planned together with the cancer team.

Anaesthesia type and hospital stay are confirmed at consultation and the pre-operative anaesthesia assessment.

What does recovery look like?

Recovery runs in stages: rest and a support garment first, normal activity over the following weeks, and scars that mature over 12 to 18 months.

• First days. Soreness, swelling and a tight, heavy feeling across the chest are expected. A support bra or garment is worn as advised; in Dhaka’s heat it can feel warm, so keeping incisions clean and dry matters.

• First weeks. Light daily activity resumes. Heavy lifting and strenuous exercise wait until the surgeon clears them.

• First months. Swelling settles and the breast shape becomes clearer. Implants often sit high and feel firm at first, then soften and settle lower.

• 12 to 18 months. Scars change from pink and firm to paler and flatter as they mature. This is normal healing, not a complication.

For a week-by-week view, see the breast surgery recovery timeline.

What are the risks of breast surgery?

Every breast operation carries general surgical risks, such as bleeding and infection, plus risks specific to implants, reduction or lift.

Risks are named before surgery, not after. General risks include bleeding or a collection of blood (haematoma), infection, poor wound healing, changes in nipple or skin sensation, asymmetry and scars that heal thicker than expected. Risk is higher in smokers and in patients with a high body mass index. Careful assessment, stopping smoking and good aftercare reduce them.

Implant safety

Breast implants are not lifetime devices. The US Food and Drug Administration (FDA) advises that further surgery becomes more likely the longer implants are in place. Silicone implants need monitoring for silent rupture, sometimes with ultrasound or MRI. A rare lymphoma called breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) can develop in the scar capsule around an implant. It is not breast cancer, is linked mainly to textured implants and is usually treated by removing the implant and the surrounding capsule, with further treatment in some cases. Some women also report symptoms such as fatigue and joint pain, sometimes called breast implant illness. The FDA has not found definitive evidence that implants cause these symptoms, but some women improve after removal. These points are discussed openly before any implant surgery.

Scars in South Asian skin

People with darker skin, including South Asian patients, have a higher tendency to form thick (hypertrophic) or keloid scars, and the chest is one of the most susceptible areas. This does not rule out surgery. It means incision placement, careful closure and scar care matter more, and all three are planned before the operation.

Breastfeeding after breast surgery

Breastfeeding is possible after many breast operations. After reduction, the evidence suggests the chance of breastfeeding is higher when the tissue under the nipple is preserved. With implants, many women breastfeed, though exclusive breastfeeding may be less likely. Women planning future pregnancies should raise this at consultation. The evidence is reviewed on what the evidence says about breastfeeding after surgery.

How much does breast surgery cost in Bangladesh?

Breast surgery cost in Bangladesh varies by procedure. Ranges for each are published in the pricing guide, and the final figure follows assessment.

Augmentation, reduction and lift sit in different price bands; technique, implants, anaesthesia and hospital stay drive the difference. Published ranges and the factors behind them are set out in the 2026 breast surgery pricing guide.

Dr. Mughni’s approach to breast surgery

Dr. Mughni’s approach rests on two things: an honest conversation about scars and long-term results, and preserving the patient’s natural body structure.

Many patients come to a first consultation hoping for surgery without a visible mark. Dr. Chowdhury Rashedul Mughni addresses this at the start:

“The first step is to accept that, without scar no surgery is possible. Post-surgery weight management is very important.”

Weight gain or loss after surgery changes breast shape, and a result is maintained by the patient as well as created by the surgeon.

On proportion, Dr. Mughni adds:

“I prefer preserving cultural features and natural body-structure, for rhinoplasty and also for other body reshaping surgeries. Always a pre-operative discussion with the patient is a must to task for me.”

In clinic, the scar is discussed before implant size, photographs or dates. Where it will sit, how long it will look pink and how the patient’s own skin is likely to heal are all covered first, because that conversation shapes every other decision. Implant size, reduction volume and lift position are then planned to suit the patient’s frame, not a fashion trend. The aim is to restore form and comfort so that the breast looks and feels like part of the body.

What happens at a breast surgery consultation?

A consultation is an unhurried assessment: your concern, your health, an examination, your options and their limits, then time to decide.

1. Your concern and goals. Many women find breast concerns hard to raise. Heavy, small, uneven or changed breasts are common and treatable, and the consultation is private.

2. Health history and examination. Dr. Mughni reviews medical conditions, medicines and pregnancy plans, then examines and measures the breasts and chest.

3. Options and honest limits. Suitable procedures are explained, including the scar each one leaves, the main risks and the realistic result.

4. Investigations. If surgery is planned, pre-operative blood tests and an anaesthesia assessment are arranged. These are billed separately by the diagnostic centre.

5. Cost and time to decide. The final cost is confirmed after assessment. There is no pressure to book on the day.

6. Follow-up. After surgery, follow-up visits check healing, scars and shape.

Why patients choose Dr. Mughni for breast surgery

Dr. Mughni is a plastic, aesthetic and reconstructive surgeon with more than twelve years specialising in plastic surgery and over two decades of surgical experience.

His qualifications include FCPS, MRCS and MRCSEd (Royal College of Surgeons of Edinburgh), and he is registered with the Bangladesh Medical and Dental Council under BMDC reg. A 41174. He serves as Office Secretary of the Bangladesh Society of Aesthetic and Plastic Surgeons (BSAPS) and is a member of the International Society of Aesthetic Plastic Surgery (ISAPS). He was previously Assistant Professor of Plastic Surgery at BIHS General Hospital and worked as a surgeon at BIRDEM General Hospital for over a decade. He is now Director of Medical Services at EKAGRA Health.

His full background is on the page about Dr. Mughni’s qualifications and philosophy, and patients’ own accounts are collected under patient reviews and experiences.

How to choose a breast surgeon in Dhaka

Whoever you consult, check the surgeon’s BMDC registration, postgraduate surgical qualifications and membership of recognised societies such as BSAPS or ISAPS. Ask who will operate and where, and how complications are handled. Clear answers about risks, scars and limits are what you need to decide.

Clinical photographs are taken only with the patient’s consent and are not shared publicly without it, and faces are blurred.

Where is breast surgery performed?

Consultations take place at Dr. Mughni’s two chambers in Dhanmondi, and surgery is performed in an operating theatre at the chamber or admitting hospital.

Breast consultations and surgery are available through both chambers:

• EKAGRA Health, Wahab Point, Floor 7, House 13, Road 2, Dhanmondi, Dhaka 1209 (primary chamber). Implants add their own long-term considerations. Safety depends on proper assessment, controlled health conditions, an experienced surgeon, a properly equipped operating theatre and careful aftercare.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Will breast surgery leave a scar?

Yes. Every breast operation leaves a scar. Incisions are placed in the fold under the breast, around the areola or in a vertical or anchor pattern, depending on the procedure. Scars usually take 12 to 18 months to fade and flatten. South Asian skin can form thicker scars, so scar care after surgery matters.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

How long is recovery after breast surgery?

Most patients return to light daily activity within the first weeks, while heavy lifting and strenuous exercise wait until the surgeon clears them. The exact timeline depends on the procedure, so a personal recovery plan is given before surgery.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Do breast implants need to be replaced?

Breast implants are not lifetime devices. Silicone implants need monitoring over time, sometimes with ultrasound or MRI.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

Can I breastfeed after breast surgery?

Many women can breastfeed after breast surgery, but it depends on the procedure. Reduction techniques that keep the tissue beneath the nipple intact give a better chance. Women with implants often breastfeed too, although studies suggest exclusive breastfeeding may be less likely. If you plan future pregnancies, discuss this before surgery.

</details>

<details class="wp-block-details mdwp-faq-item is-layout-flow wp-block-details-is-layout-flow">

What is the difference between a breast lift and breast augmentation?

A breast lift raises and reshapes a breast that has dropped, but it does not add volume.
